    my brother did that - he only took off a little bit (with the tip of his fingernail) and it seemed to work fine, but just to make sure he bought a tube of thermal paste

    AFAIK it's dangerous to use both a thermal pad & paste together. One or the other is OK.

    It depends on the paste. You can get paste which is more or less the same material as the thermal pads, so it never sets, that is probably ok. But if you use a setting paste with a thermal pad you get a pad which doesn't soften as much as it should, and a paste which doesn't set. The only solution is to remove tha pad completely and use paste (or get a new pad).
