Icon Packager by far.
I am not all too certain if it can actually create icons, however it can extract icons from any available software you may have on hand or in a sense create icons from .bmp images that you may have as well. Editing in general, is limited with the program. It best function is applying entire "packages" of icons to your system.
http://www.wincustomize.com/ for details. Personally, I think that this is one of Stardock's better programs. Aside from being an Object Desktop component, it pairs equally well with StyleXP. XP themes designed/created by http://www.lightstar1.com/ use this software and the icon packages ditributed through WinCustomize.

well i have tried microangelo and kind of like it and as for icon packager i didnt like it especially since its from stardock
