That skin is my favourite. I too use that skin. The name of the skin is Noia 2.0 (eXtreme).

That music addon looks familiar to me. I also had used it once, but don't remember the name of the extension.

It's the FST vBulletin Dark (BP-Grey) style. Scroll down to the bottom left corner of your FST page to change the style.
