The title:

Holden keeps having this dream where there are kids running and playing in the rye. This field has a steep cliff on one side and he has to stand there to prevent them from falling off, he has to catch them.

The whole story is in the past (telling it in a mental hospital) . The dream is maybe a symbol of him trying to save the "worlds kids'. Of course he is having a breakdown through out the story. Just read the book it is good.
