Easy CD-DA Extractor (http://www.poikosoft.com/)
Comes with Easy CD-DA Extractor, Easy Audio File Convertor, and Easy Audio CD Maker, all in one. :D
Only problem is it isn't free, but I'm sure that won't be a problem.

Nero (http://www.nero.com) with the audio plug-ins can rip to many formats.
CDex (http://cdexos.sourceforge.net/) is a free cd ripper that's highly rated.
