Install DVDInfoPro and ImgBurn, burn an image with verify turned on, then click the button to view DVDInfoPro's performance graph. Don't put too much faith in Nero's verification. If the media is poor and Nero has trouble verifying but does so eventually after slowing to 1x speed or reading multiple times, then it still passes for a good burn even though future data extraction from that burn is likely to be a problem.
Go back and check some of your previous successful burns on the media you're currently using and try to rip the items back to HD. Poor media often fails even though it appears to be a good burn (DVD plays in standalone, audio CD plays in car etc).
