I know a guy who puts out hand rolled cigarettes with the tips of his index finger and thumb. He doesn't do it to look cool/mental or anything as he does it when no one (except me, surreptitiously ) is watching him. So I don't think it will hurt much. The tobacco is probably too loose to cause much pain or something else.
