I don't change my desktop theme everyday (or ever, really...) and I use it. It just gets rid of XP's ugly "Standard Buttons" and "Address bar" in explorer.

Just make sure you delete the shortcut in you're "Startup" folder and set the prog to TB Mode only 'cause it will try to manage your themes at first. Other than that...it's just a shell addon that doesn't even have to run in the background.
