On sites such as 1&1 hosting, it says 12,000 MBit At Your Command.

Does that mean upto 12GB data tranfer per second?

no your only as good as your isp
like saying I'm on a 56k but I'm download off a t1 how come my speeds are only 5kb
speaking of fast ass connections, check out the future of isps with ftth
    http://www.lonestarbroadband.org/technology/ftth.htm

    How fast is FTTH?
Fiber optic cables can currently carry information at speeds greater than 2.5 gigabits per second. Residential/business FTTH typically offers speeds from 10 mbps to over 100 mbps, which is a hundred times faster than most cable or DSL service and over twice as fast as a T3 connection.

    No it means that they have a 12Mbit/s input/output capacity,they don't mean per-website

Your site is sharing this 12 Gigabites/s with all the other sites on their servers. The maximum bandwidth anyone downloading from your site can get depends on the traffic at the time.
